What Is SBA Form 641?
SBA Form 641, U.S. Small Business Administration Counseling Information Form is a form used for surveying and counseling the clients of the Small Business Administration (SBA) Resource Partners, including Small Business Development Centers, SCORE, and Women's Business Centers. Those organizations provide training and counseling to existing or potential small business owners through SBA-funded grants or cooperative agreements.

The SBA counseling information form is filed by potential or current small business owners looking for business counseling services from the SBA or an SBA Resource Partner and is a means of gathering information about their business, background, and required assistance.
By signing the form, the business owner consents to receive information mailings and to cooperate if selected to participate in SBA surveys. The SBA Form 641 also includes a waiver of claims against the SBA personnel, Resource Partners and host organizations that may arise from their assistance. The form should be submitted directly to the counselor providing the services.
Resource Partners will then forward the information to the SBA by either manually submitting their counseling and training data or using software to upload *.xml files. SBA partners are required to provide this data quarterly. The SBA will then compile the data to evaluate counseling and training performance.
The second form used for reporting training provided to entrepreneurs and potential business owners is the SBA Form 888, Management Training Report. The form is used to collect and report information on traditional in-person training (with two or more clients in attendance) and online training.
