What Is SBA Form 3315?

SBA form 3315, Nomination Form for District Office Awards is a form filed when applying for the Small Business Administration (SBA) District Office awards. Any owner of a small business operating for at least three years can be nominated for this award. Receiving SBA assistance does not prevent the business from being nominated and awarded.

Each district office accepts nominations for various awards. The winners of the district office and state-level awards will be selected for the regional level. The regional winners will be selected for the national award level. This SBA nomination norm can be used to elect a team of up to three co-nominees. In this case, each co-nominee should file the SBA Form 3300, Award Nominee Background Form.

SBA Awards

The SBA Form 3315 is a part of an award-related set of SBA forms that consists of four forms in total. The three other SBA nomination forms include the following:

  1. SBA Form 3301, Nomination Form for Small Business Person of the Year is used to nominate an individual for the Small Business Person of the Year. Any legal resident of the United States that operates, owns or bears principal responsibility for a small business for a minimum of three years may be nominated. Individuals receiving SBA assistance are also eligible for this award. The nominees are evaluated in the following criteria: growth in the number of employees, an increase in profit, response to adversity, contributions to community-oriented projects, overall innovativeness, and staying power.
  2. SBA Form 3300, Award Nomination Form is filed by each individual nominated for an award, including team members and official representatives of nominated businesses. This form requires personal information on the nominee, as well as the information about their business and an accurate criminal record. Positive answers will not disqualify the nominee, but providing false information will and might lead to penalties. The completed form must be submitted with the nomination package.
  3. SBA Form 3300, Award Nominee Background Form should be filled by any individual nominated for an award, including members of nominated teams. The form is used to determine the nominee's eligibility for an award and is submitted with the nomination package as noted in the National Small Business Week Award Nominations Guidelines.
